*Fill in the Consultation Response Form found at the back of the Shaping Healthcare for Lincolnshire Document (which can be downloaded from www.lpct.nhs.uk).
* Write to: Public Involvement Team, Lincolnshire PCT, Cross O' Cliff, Bracebridge Heath, Lincoln, LN4 2HN.

* Call The Public Involvement Team (Tel: 01522 515333).

* Email the PCT at public.involvement@lpct.nhs.uk

* Attend the public information event on the proposals at the Guildhall Arts Centre, St Peter's Hill, Grantham, from 2pm to 7pm on Tuesday, June 12.

Closing date for feedback is 5pm on Thursday, August 9.
